UDSv4 Specific Instructions

For participants who had previous UDS data and are completing UDSv4 for the first time:​

  • For PACKET, use the code I4 - Initial UDSv4 Visit Packet.
  • For VISITNUM, please continue with the next sequential visit number for the respective participant, do not restart visit numbers.
  • VISITNUM and VISITDATE must be higher than the last UDSv3 visit submitted for the respective participant.

For newly enrolled participants:​

  • For PACKET, use the code I - Initial Visit Packet.
  • For VISITNUM, use the existing visit number scheme for your center.

Associated Modules [LBD, FTLD, DS]:​

  • PTID, VISITNUM and VISITDATE of the associated module packet should exactly match those of the respective UDSv4 packet.

Even when the visit is split into multiple days, the same VISITNUM and VISITDATE MUST be used for the header variables in both packets (UDS and module). Guidance is to use the date that form A1 is filled as the VISITDATE for both packets. Individual form dates (FRMDATE[XX] variables) can be different depdending on when they are completed.

  • If submitting an module that is associated with a UDSv3 visit (except for Down Syndrome module and Form B1a), you must use the legacy submission system.
