udsv4 will no longer include the z1x form as a data collection instrument. variables to collect language, submission status for optional forms, and reason for not submitting have been moved to the header for each individual form. please refer to the udsv4 data element dictionary for details on these variables. nacc developed a guidance for udsv4 submission document to indicate which forms are absolutely required for submission

the redcap forms were carefully developed by members of the edcs workgroup in collaboration with nacc and the clinical task force, with substantial attention to detail and alignment with program-wide standards.

if your center elects to make custom modifications to these forms, please be aware that such changes may introduce unexpected issues or inconsistencies. the center is responsible for validating, maintaining, and troubleshooting the functionality and integrity of any customized forms.
