COVID-19 Module Downloads
Latest Release: 1.0.0 (10/13/2026)
NACC originally released COVID-19 Version 2 REDCap files in 2022. This new set of files represents some updates, including the Form Header instrument and the Follow-up Visit Packet. If a Center was using the REDCap files we provided in the past, they would need to add new variables/instruments to their existing project. Please find the details below.

Submission Information
- The new system only accepts version 2 of COVID F2 and F3 forms (Form F1 is no longer accepted by NACC).
- Multiple initial visit packets can be submitted for a participant (if they are already collected), but the newly released follow-up form should be used going forward.
- FVP packet submission requires an existing IVP packet for the participant.
Packet-level Information
Unlike other NACC modules, variable names vary between initial and follow-up visit packets. Some questions asked in the follow-up packet have the initial packet variable name with '_f' as the suffix.
Survey View
Both F2 and F3 can be administered as a survey. The questions at the top of each form should only be seen and answered by clinicians, however. And then the instructions for survey participants only need to be seen by survey participants, not project users. To that end, the questions at the top of the form (for clinician use only) have the HIDDEN-SURVEY action tag applied to them. And the survey instruction fields have the HIDDEN-FORM action tag applied to them.
Changes to COVID-19 Module Header Variables
There have been some minor changes to the COVID-19 module header variable names to support the data ingest through the new NACC data platform. The following table lists COVID Initial Visit Form v2 changes from the 2022 release. You can also download this table as an Excel document.
